Rome airport transfers come down to four numbers. From Fiumicino (FCO, 32 km west of the centre): €14 for the Leonardo Express to Termini, €8 for the FL1 regional to the western and southern stations, €6–7 for a shuttle bus, €55 flat for a licensed taxi to anywhere inside the Aurelian Walls. From Ciampino (CIA, 15 km south-east): €2.70 for the Trenitalia bus-and-train combination, €6–8 for a shuttle bus, €40 flat for a taxi. Everything else on this page is about which of those fits your arrival time, your address and your luggage.

Fiumicino by train: Leonardo Express or FL1
The station is inside the airport, a covered walk from Terminals 1 and 3 past the shops. Two services run from it, and they are not interchangeable.
Leonardo Express is the non-stop shuttle to Roma Termini: 32 minutes, €14, every 15 minutes with a few 30-minute gaps, from Fiumicino 05:38–23:27 and from Termini 04:50–23:05 according to Trenitalia. Four people travelling together can buy a "Minigruppi" ticket for €40, and a carnet of ten costs €115. Children under 4 travel free, and one child aged 4–11 travels free with each paying adult. ATAC and Metrebus city tickets are not valid. At Termini the train uses the far platforms, so allow 10 minutes to reach the metro or the taxi rank.
FL1 is the regional line that runs from the airport through Roma Trastevere, Ostiense, Tuscolana and Tiburtina and on to Fara Sabina and Orte. It costs €8 to any station inside the Rome ring (Trenitalia's Tariffa 1 Ter; €11 to Lazio stations outside the ring), runs every 15 minutes on weekdays and every 30 on Sundays and holidays, and takes 48 minutes to Tiburtina at the far end of the run, with Trastevere and Ostiense reached well before that. It does not go to Termini. If your hotel is in Trastevere, Testaccio, the Aventine, Prati or the Nomentano district, FL1 plus a short taxi or tram beats the Leonardo Express on both price and time. Validate a paper ticket at the airport turnstiles; the Trenitalia review explains the €100 penalty for an unvalidated regional ticket.
Tickets for both are sold on the Trenitalia app and site, at the station machines and desks, and at the tobacconists in the arrivals halls. Tiqets and Headout sell the Leonardo Express as a voucher too; the comparison below shows what that costs against the €14 counter price.

Fiumicino by bus
Five bus companies run to Roma Termini from the bus station outside Terminal 3, and Aeroporti di Roma lists their stands: SIT Bus Shuttle at 16, TAM at 17, Terravision at 18, Rome Airport Bus by Schiaffini at 19 and Cotral at the T1 stop. The trip takes about an hour in normal traffic and longer at the evening peak, which is why the train wins for anyone with a tight connection. Buses stop on the streets flanking Termini; the exact stop and any intermediate stops are on the operator's site and your ticket. Tickets are sold at the bus station desks, online and, according to ADR, on board depending on the company.
On the platforms the same rides are €6–7, prices checked regularly. GetYourGuide's Fiumicino–Termini bus is the most-reviewed transfer product in Rome and the cheapest; the SIT and Terravision tickets on Tiqets and Headout are the same buses at €7. Buy the voucher for the certainty of a seat in the peak season, or just walk to the stand.

Aeroporti di Roma lists night buses to Termini, and Cotral runs its Air Terminal night bus between Termini/Tiburtina and the airport for €5 (€7 bought on board). Check the operator's own timetable for anything after 23:30; the platforms' vouchers are for daytime schedules.

Taxi: the flat fares and how to use them
Official taxis are white with a "TAXI" sign on the roof, and at Fiumicino they wait at the ranks outside Arrivals. The Comune sets flat fares for the airport runs and Aeroporti di Roma publishes them: €55 between Fiumicino and any address inside the Aurelian Walls, which covers the whole historic centre, Trastevere, the Vatican side and Termini; €60 to Tiburtina station; €50 to Ostiense; €55 to Ciampino airport; €130 to the port of Civitavecchia. The flat fare includes every supplement: luggage, night, Sunday. For addresses inside the GRA ring road without a flat fare the meter applies but cannot exceed €80, and the minimum for any run from the airport is €35. Taxis licensed by the Comune di Fiumicino (not Rome) also work the ranks, on the main road outside Terminals 1 and 3 and by the station; runs into Fiumicino town have a €25 minimum.
From Ciampino the flat fares are lower: €40 to the centre inside the walls, €40 to Ostiense or Tiburtina, €55 to Fiumicino, minimum €25 elsewhere.
Two rules. First, use the official rank and ignore anyone offering a car inside the terminal: ADR's own page warns that vehicles at the exit may be unlicensed and charge more. Second, say the flat fare before you get in ("cinquantacinque, tariffa fissa") and ask for a receipt (ricevuta). For four people with luggage the €55 taxi is €13.75 a head, which is cheaper than the Leonardo Express.

Private and shared transfers
A pre-booked private car costs more than a taxi and buys three things: a driver in the arrivals hall with your name, a fixed price agreed in advance for addresses outside the flat-fare zone, and a vehicle sized for your group and bags. On GetYourGuide the private Fiumicino transfer starts at €75 per vehicle and the city-centre-to-Fiumicino transfer at €88 per vehicle; the any-airport private transfer is €180 per vehicle, the dearest of the three. All three are priced per group, not per person.
For reference, the Rome NCC firms we review publish similar numbers direct: Cab Shuttle Taxi lists Fiumicino to the centre at €60 for a Mercedes E-class sedan, €80 for a V-class and €90 for an 8-seat van, with tolls and ZTL permits included and free waiting up to 90 minutes for a delayed flight; the Cab Shuttle Taxi review and the LUXURBE review cover what the sites do and do not publish. Tiqets' shared minivan to your hotel is the middle option at €27.67 per person: door to door, but you wait for the other passengers and it is rated lower than anything else on this page.
Ciampino: bus, Airlink or taxi
Ciampino has no railway station of its own. The choices are the shuttle buses to Termini (ATRAL, SIT Bus Shuttle and Terravision, listed by ADR, about 40 minutes), the Trenitalia Ciampino Airlink and the taxi.
Airlink is the budget answer: a Trenitalia bus leaves stand 6 outside the terminal every 20 minutes, runs non-stop in 10 minutes to Ciampino railway station, and a regional train takes you on to Termini. The combined ticket is €2.70 (the €1.20 bus leg added to the regional fare), bought on the Trenitalia app or machines with "Ciampino Aeroporto" as origin or destination. Ciampino station also has direct regional trains to Frascati and the Castelli Romani, useful if you are heading straight out on the Castelli day.
The shuttle buses cost €6–8 on the platforms and go direct to the streets beside Termini; ATRAL and ATAC lines also run to Anagnina at the end of Metro A.

Timing the transfer around a day trip
If you land in the morning and want to use the afternoon, the Leonardo Express plus a left-luggage drop at Termini gets you into the city by mid-morning; Rome in a day has the half-day loop. If you land late, take the taxi or a private car and save the trains for the Tivoli or Orvieto day. Cruise passengers going straight from Fiumicino to the ship should read the Civitavecchia page: the €130 taxi flat fare, the €11 through regional ticket via Trastevere or Ostiense and the port shuttle are all there. Leaving Rome by the Frecciarossa after a day trip and flying the same evening is possible from Termini via the Leonardo Express, but give yourself 45 minutes at Termini and two hours at the airport.

Verdict
For Fiumicino, the Leonardo Express is the default if you are staying near Termini, FL1 if you are staying west or south of the centre, the €6–7 bus if money matters more than 30 minutes, and the €55 taxi for any group of three or more with bags. For Ciampino, the €40 taxi or the €2.70 Airlink, depending on whether you are three people or one. Book a private transfer only for late arrivals, big groups or addresses outside the flat-fare zone. Rome airport transfers are not where your day-trip budget should go: save it for the departures board.
